Concurrent Credit Forms

Concurrent Credit allows you to enroll in concurrent credit courses, paving the way for a dynamic educational experience that bridges high school and college. Our streamlined process ensures a hassle-free application, empowering students to embark on a journey of advanced learning and college credit acquisition. 

Students who plan to enroll in both General Education and Teacher Residency concurrent credit courses must complete and obtain signatures on both Pre-Registration Forms. However, only one High School Concurrent Student Memorandum of Understanding (MOU) is required.

FERPA Release Form

FERPA Release Form (Allows you, the student, to give permission for specific information about your academic records to be shared with individuals you choose (for example, a parent/guardian or your concurrent credit teacher. Completing this form is completely optional, but it may be helpful if you want someone else to be able to discuss your academic information with your concurrent credit teacher on your behalf.)
